Go to the pediatrician and get nystatin... terrible diaper rash is almost always a yeast-type infection and requires an antibiotic ointment. Hope she feels better!
My daughter who is 14 months has a terrible diaper rash. My neighbor suggested dyprotex that she used on her youngest who is now 6. I have searchec AND searched and can not find this stuff. Do they even make it anymore? I have read there was some issues with this stuff so maybe they took it off the market.

I used Bag o' Balm when the kids were little. It comes in a green tin with a bag inside. That stuff worked wonders! Hope you're able to find something that works for your little one!
Terrible diaper rashes are almost always yeast infections. Which need an antifungal cream to get rid of them (usually a lotrimin type cream, but sometimes they'll use other antifungals... depends on the type of infection and the severity of them).
Recurrent yeast infections are often a sign of an allergy. The allergy makes the skin degrade, and then an infection moves in on the degraded skin. Yeast is the most common, but bacterial infections are also common. Many kids are allergic to certain brands of diapers. My own son could wear any diaper, but he was allergic to every brand of wipe... the more "sensitive" the wipe, the stronger the allergy. Oy. Talk about backwards! Anyhow... once I switched to damp paper towels instead of wipes, the yeast infections went away entirely. Took a couple months to figure that one out. Poor little guy!
*** By went away... I mean AFTER using the antifungal cream, it didn't come back again. But as long as I was using wipes, he had yeast infection after yeast infection.
How to tell if it's a yeast or bacteiral infection vs normal diaper rash?
1) Barrier creams don't fix the problem.
2) Red and inflamed, fungal infections also often have a bumpy characteristic, and they often bleed when they get very, very bad.
How to tell if it's an allergy degrading the skin?
Process of elimination. Save some $ and use wet papertowels for awhile (1-2 weeks) to rule out wipes. Change diaper brands. If you cloth diaper, change detergent. Any allergy is going to come on stronger in a hot and moist environment... because the allergens transfer easier. So if you or your baby are allergic to your detergent... it will show up in socks and underwear before the rest of your skin.
Ok, terrible diaper rash 101
Step ONE cool bath with baking soda
Step TWO pat dry (with paper towel) and allow baby to air dry
Step THREE Mylanta Original (no flavor) dab with cotton on effected area
Step FOUR Cover Mylanta with vaseline or zinc cream
Step FIVE change diaper often, do not use wipes (soak paper towels in warm water with baking soda and a touch of mild soap) with alcohol
Step SIX repeat 3 4 5 w/ each diaper change and 1-5 at least 3 times a day esp after BM.
If you follow this you will see results in a few hours and you should see a dramatic change w/in 24hrs if you do NOT see this change you should contact a doctor it may be a yeast rash.
