Could she be allergic to her diapers? I use 7thgeneration. My other thought is an yeast infection that creams won't help.
My daughter is almost two. Super quick background...she has Celiac disease, has been GF for over a year, and has had severe trouble with constipation to the point of anal fissures.
We've got the constipation fairly under control now, and she drinks plenty of water. But she still almost ALWAYS has an excruciating diaper rash...to the point of purple, lost skin, cracks, and bleeding.
I change her diaper WHENEVER it's wet or dirty...immediately. Always have. I have used Triple Paste, Bordeaux's (?) Butt Paste, and Vaseline. NOTHING seems to help. I just want to get it healed...and can't figure out why it keeps coming back.
If anyone has experienced this or has any helpful advice, that would be great. The doctors just keep recommending Triple Paste or Vaseline...they don't help. Nor does Desitin, blah blah blah, other creams...I'm really hoping someone has some non diaper rash cream advice!! I hate changing her diaper, because it's a screaming, crying mess and I feel so awful. I always say "Mommy's not trying to hurt you" and my little talker has turned this into "Mommy try to hurt you!" while crying during diaper changes and I feel just awful.

Wanted to update you all...she has a yeast infection. She's on some prescription cream now...she's been on it for about three days, and it's finally beginning to heal. I am two things...so pissed off that how many doctors didn't RECOGNIZE this for what it was (and her doctor didn't even check...I called, said "I think it's a yeast infection" and she just faxed in the script for me...) ...and on the other side, I am SOOOO relieved that we're finally getting this under control.
She's feeling so much better, and so am I...she's not screaming and throwing herself into the air when I change her diaper now!

Hmm... her problems with constipation makes you wonder if she KNOWS pooping will cause a diaper rash... so she withholds it... causing the constipation!

My Nan always swore by corn startch. Depending on the severity, I use it by itself or mixed with a&d for my son or desitin for my daughter.
A client of mine got this recommendation from her pediatrician (we call it Magic Cream at our house!).
Equal Parts: (Most of these things you prob. have on hand)
(it doesn't matter if you use the ointment or cream form, just use what you have)
Butt Paste
Antibiotic Ointment or Cream
Hydrocortizone Cream or Ointment
Anti-Yeast Cream (like Monostat or Tinactin)
Mix it all together and apply after each change. It truly was a life saver.

Are you sure is a diaper rash and not a yeast infection? My son had a diaper rash not as severe as what your describing, but once I used some antifungal cream mixed in with a barrier cream it started clearing up w/in a day.
In my opinion, with the extremeness (if that's a word) of what you are describing I would take her in to get some prescription medication from her pediatrician. I know my mom had to take my nephew in on her watch.... Poor baby and Mommy--can't be fun.
---OOps I just reread your question I can't believe the dr's aren't prescribing more! Seems obvious she needs something else, and in my experience from other Mom's stories never hard to have them prescribe something. Wow--new pediatrician or more demanding maybe!
Baby powder and let her air dry for a few moments before putting the new one on. Also a hair dryer on it's lowest coolest setting on her tush to dry her up. Just swing it back and forth over her rear. My daughter loves the hair dryer in the winter time, I use it to warm her up fast between bath and getting jammies on.

PS: If it is a yeast infection, yogurt is a safe way to help. Add it to her diet and see if it helps any, if so you can decide if you want to try an OTC medication for yeast infections.

That sounds terrible! Poor baby. I would get a second opinion right away. These drs don't seem concerned about the rash, which doesn't sound like diaper rash at all! Take her to another office and demand some relief for your daughter. I would be in that office every other day until something they do for her starts to clear it up. Good luck!
Ask your pediatrician to a referral to a dermatologist. None of us are going to be able to diagnose your daughter, and one thing could make it worse over the other. I had been using a special blend compound perscribed by my ped for awhile on my daughter and it wasn't working. When she finally referred us to a dermatologist he told me that the compound (I brought it with me) was actually exaserbating the problem and could cause prolonged redness. FWIW my daughter's rash did end up being a yeast infection, and vinegar baths combined with OTC athlete's foot cream cleared it up. (vinegar baths were 1c vinegar in a tub of water.) I also swear by baking soda baths for toddlers, they clear up almost everything. :) Good luck!

I would not use the paste's you've listed. I was on a site looking at a toxicology report the other day and Bordeaux's BP was at the very bottom of the (fail) list. (I was surprised by that.)
I have had really good results using the product I'm recommending.
I also think coconut oil is a good suggestion.

One thing that helps my son is to use soft washcloths just dampened with water. That will probably help her immediate pain as you try to clean her up. There are so many chemicals in wipes that just burn if they're on raw skin.
My son was hospitalized months ago and the antibiotics gave him terrible diaper rash. They gave us stuff they use. I doubt you can buy it at the store, but I'd insist to your doctor that you need something stronger than a recommendation. You probably need a prescription. She's got enough issues with pooping, she doesn't need diaper rash too. Also, have you ever tried plain cornstarch. Sometimes that area is just too damp so drying up can help. Best of luck!
My son had this same type of diaper rash every time he cut a tooth... Turned out he would get a yeast infection on his skin each time. We had to get a prescription ointment to use 3-4 times per day. With the in between diaper changes we found a powder called Mexana (I think one of the few places that we found that carries it was Walgreen's) that is similar to Gold Bond without the menthol that burns on raw skin and the powder was very healing and would help keep his skin dry. We would put this powder on then apply a combo of Neosporin and A and D ointment. This would heal him up every time! Once we figured his rashes out, we could see them coming and keep them from turning into the situation you currently are describing! I feel for you because I was there... We also hung his little hiney over the sink, used paper towels (softest we could buy) and warm water and baby soap instead of baby wipes. They didn't seem to burn quite as bad. Was a little messier but well worth the pain it saved him :)

Besides giving her a good probiotic daily, it sounds like you need to eliminate something else from her diet--probably the root cause of the rash. Think about what she eats regularly or craves and that may be your answer. My skin issue trigger is the nightshade family--wht potatoes, peppers, eggplant, etc.
my kids dr said to always let the hiney air out-always did the trick-yea can get messy-but really helped alot-nothing on the bottom for a while-plus just plain yogurt spread thinly on the behind really helped alot-good luck
Go to an animal feed store and ask if they have Corona. It is used on animals but works on humans too. my sons grampa used to use it on him and it would clear it up no problem. then go to the grocery store and get yourself a bottle of Corona. :) hope she feels better soon!!!!
Other then it possibly being yeast, and asking for a prescription and diagnosis...I don't have much advice!! Well, expect I DO have diaper cream advice. (I know you wanted other advice!) Look into Weleda Calendula Diaper Care Cream. It's not a typical diaper cream. It has calendula. It's not just to block moisture, it actually has soothing extracts that heal. Anyone I've known whose had bad rash problems, I recommend this cream to. I've used it on a number of things...burns, sunburns, etc. It's really amazing and soothing.
Please, don't use any kind of diaper "powder" on her rash. Powders are just awful, and id it's yeast...it will simply feed the yeast. Also, corn starch will feed the yeast, as well. It will make it worse. If it's not yeast, cornstarch shouldn't hurt.
Aquaphor. Its the miracle to all rash, chapped or anything of the like. its soothing when put on as well. Seriously give it a shot. I swear by it

We had a child in my child care center that had a diaper rash that was so bad it was bleeding. We had used the above mentioned items and nothing changed.
My partner has been in the child care business since his mother opened this center when he was a child. His wife took it over and retired from it, he told me to use anti-acid on the rash. I laughed at him, thought he was joking. He wasn't. He told me it was an old wives remedy. I asked the mom if she minded if I tried, she said she was at the point of trying anything.
I had some Mylanta or Maalox in my desk. He told me to take cotton balls and dab them in the liquid then blot it onto the girls hiney. Of course you use a new cotton ball each time but I still poured some into a bowl first. I dabbed it on and she was fussy a bit, I wondered if it burned but she didn't scream.....
It was gone in a few days. I swear it to be true. I was amazed. I have never seen a diaper rash so bad as that one and it was gone in a few days.
My child gets very bad diaper rash. When it's bleeding or she has sores I put on an antibiotic cream prescribed by the doctor for foliculitis - bactoban I believe it is called. On top of that I put the A+D cream and on top of that I put the Triple Paste. I put it on about 1/2" think so no moisture can touch her butt. I do that every single time. You might try changing her diaper every couple of hours regardless of whether she has urinated just so the paste gets reapplied until she can heal. If she is teething the urine can get more acidic and the second it touches the skin it burns the skin. Same with if she were to have diarrhea. I have been through this several times with my daughter. She gets HORRIBLE diaper rash. She will be three in June; we just had an episode of this last month.
Oh and lately I have been drying her off with baby powder before applying all of the creams and I have given her some baking soda baths too.
corn starch worked best for my son. creams and ointments kept the area to damp, the corn starch kept it dry. have you thought about starting to potty train her? getting her out of a diaper and into breathable panties will help a lot, too. i used to buy big boxes of corn starch and then put it into shakers to sprinkle on the kiddo. good luck!
My son had some bad diaper rash from being on antibiotics. Well, we thought it was bad, but it was nothing like what you are describing. Without much prodding, the pediatrician gave us an antibiotic ointment that healed it right up. I would make an appointment and not leave without something.... otherwise, I guess this would be good motivation for potty training :)
I'd try the recipe for "Magic Paste" another poster mentioned below.
Also...my grandma's miracle cure: Crisco. The plain white kind in the blue tub. Slather it on generously. It works like a charm.

You are using what I always did with respect to 'topical' ointments...
But one thing that always helped for ANY kind of sore bum with my kiddos was a warm baking soda 'soak' in the tub...
I would take a smallish box...and dump it into luke warm bath water...mix it up until it dissolves...and play with them in the tub til they were 'pruned'. Then dry gently with a towel...let them run free til absolutely needed a dipe...pull up...or panty...Then slather with butt paste (or similar)...and then repeat...repeat...repeat!

Get a tube of hydracortisone, and one of athletes foot cream mix them together in small bowl and apply to sore bottom and.see if that works, my grandson would never clear until a few days at my house.

My son went through a really bad bout of diaper rash where nothing seemed to work, open sores, etc. The pharmacist recommended something called Domeboro. It was AMAZING!!! It's a powder, comes in a box with about 14 little packets. You mix it with water and then soak a rag (I used a paper towel) and lay it over the rash. Rediaper over the rag. I didn't throw out the unused portion immediately as the directions say, but rather covered the container and used more later. Never "double-dipped" the rag or paper towel so no risk of contamination. This stuff was the only thing that cleared up his diaper rash! We also used Burt's Bees diaper cream, which we found better than any other diaper cream. Good luck!
I know you got a lot of answers already but wanted to add what my pedi recommended and it worked great!
Half aquaphor and half mylanta. Heat the aquaphor in the microwave for a little bit to get it really creamy and mix the mylanta in. Keep stirring until it is a thick paste. It worked on my daughters bleeding diaper rash in a matter of hours! Every time she started getting a rash I'd put the "Magic Cream" on her and it would be gone by morning. Also, have to add like a lot of other posters, have her checked to see if it might be a yeast infection. You can get OTC athletes foot cream for the yeast infection. It works really well too.
Also, as much air time as possible. They say the air will help clear it up much faster.
Turned out my daughter had a citric acid sensitivity and that's why she'd get a rash almost instantly.
GL. I know there are so many possibilities and it just plain sucks trying to rule out what might be the cause of the rash.

Sounds like she has very acidic stools and maybe urine as well. Maybe due to some meds that she's on? Not sure what dietary changes you could make to help her ph. Maybe try some probiotics or yogurt.
I'd try an antibiotic cream and let her go bare for as long as you can. Keep it dry with some cornstarch or cornstarch based baby powder.
Poor baby... I'm sure it's excruciating for her.

Are you using cloth diapers still? Either way, have you considered changing brands?
Wipe her with a wash cloth and water or the Kleenex brand paper towels and water. Don't put anything else on it to clean her. If she has open sores, put some neosporin (sp?) and then dust with cornstarch.
Also, she may have an additional food allergy you have yet to discover. Think through her diet and identify things she eats everyday or close to it. Try eliminating things 1 at a time and see if it makes a difference.
I've experienced this with 2 of my 5 children, where nothing has worked and the pediatrician recommends everything under the sun, even liquid antiantacid such as malox or even vitamin e capsules broken open. It actually was so simple of a solution for us, that I felt terrible it took so long to figure out. Baby powder. The aloe and cornstartch variety works best but lots of it. It will also probably help if you rinse her bottom instead of using wipes. No matter how unscented they are, water is always better. Good luck, hope this works as well for you as it does here!

Corn starch worked best for us. Just regular old corn starch, from the baking section at your local grocery store, sprinkled lightly after every change.
And since she's almost two, get her OUT OF THOSE DIAPERS. I bet you can tell when she needs to go (the hiding in the corner, the grunting) so take advantage and try to get her on the pot as much as possible! Let her run around without a diaper and set her on the potty at least every thirty minutes. She'll pick it up quickly. Unless she has a physical/medical condition she will PREFER going in the potty, I promise, and you will both be happier :)
Sometimes they need a perscription cream.... i think it sounds like yeast.... (if i remember correctly)
For now, i would leave her bare bottom and have her drink lots of water. No orange juice or anything remotely acidic. I hope it gets better soon :(
Have you taking her to the doc lately and had it looked at?
My girlfriend just took her little boy in (he just turned 3) he has lots of problems with his bowels, and developed a rash.
It turned out to be a staph infection. If the sores go on for too long, it can easily turn into a staph infection, which will not clear up without antibiotics.

if you are changing her diaper every so often the problem may be the diaper brand. my daughter is allergic to other diaper brands so i patiently tried different brands and see what makes her bottom comfy and rash free.
if she is constipated let her eat fruits and veggies of if she cant eat em try a juicer.. make snow cones with fruits.. this will help her bowel movement. i think soy i safe. ask her doctor soy, if shes allowed to eat yogurt then let her have it..
Have you tried Vagisil? Some horrid diaper rashes are actually a type of yeast infection. NEVER put cornstarch on a diaper rash--that just feeds the yeast infection. My son had terrible rashes and I finally had a doctor who said to try Vagisil cream. It seemed kind of weird to me, but it worked like a charm.
You have gotten lots of ideas of pastes and creams to try, I would also like to say, the wipes make a difference too, and the diapers. I made my own wipes for my kids up through potty training. I would cut a roll of Viva paper towels in half with an electric knife, put 1/2 of it in an air tight plastic canister, then in a measuring cup, put 2 cups of hot water from the tap ( you could use distilled, or boil it if you are concerned about the safety of the water on her open sores), and mix in 2 Tbsp of whatever baby wash we were using, and 1 Tbsp of Baby Oil, and then pour that in the canister over the paper towels, pull out the cardboard in the center, and that is how you start the roll, let the wipes soak up the liquid, and there you go. My kids rarely got a diaper rash, unless they got too much citrus in their diet, if they were teething, or if we used the wrong kind of diapers or wipes. When they did, though it tended to be a really bad one with sores and oozing, etc; we used Triple Paste, or Hydro-Cortizone mixed with that, and gave them lots of diaper-free time. You could try using flannel rags (cut up old reveiving blankets) instead of paper towels, and just wash them in between uses. You can look online and find even more recpes, using all natural essential oils, too.
Also, my kids could wear nothing but Pampers for the first 6 months, all the other disposables broke them out, after 6 months their skin got a little less sensitive and we could use other brands, but every baby is different. But you might try switching brands? I wonder if any of the diapers have gluten in the chemicals in the absorbant stuff???
One last thing, have you thought about trying to potty train her, so she no longer has to deal with a diaper holding chemicals and wetness and poop on her at all ?? Even if she isn't ready to night train - you can day-train her, and it will make her life and yours so much more painless!!! She can go without panties or anything when in the house and then just wear cotton panties when out and about, I would reccommend getting a little potty to go in the car with you while you are out, much easier then dealing with public restrooms or going potty outside, their little bladders can't hold it more than an hour or two at first. ( we have this and love it http://www.onestepahead.com/catalog/product.jsp?productId...) My daughter was Day trained at 22 months, night trained later at 3 when her body was ready.

Arbonne Diaper Rash Cream. It is absolutely amazing! It's all natural...and if it wasn't I would almost be scared at how well it works. Literally, huge results from one diaper change to the next. I have used Triple Paste, Desitin, Vaseline...and a couple others. (Before knowing about Arbonne.) None of them touch Arbonne. I don't use anything else now. It's is a bit more pricey ($14) for a small tube, but worth every penny...and you will need to find a local Arbonne consultant to order it. Good luck! (Their other baby products are just as good...I have used the baby wash and have the lotion.)
My kids never had diaper rash but my cousins kids had it where it bled. She finally started using a powder called Caldescene. Its in a pink bottle and its in the baby section or med section at Walgreens. Its about $6 but worth it, she swore by it and its made for this.
This was going on with my son. Think about her diet. My son's rash ended up being due to all the grapes and cherry tomatoes he was eating. What helped the best was:
1. Lots of naked time. Yes, you are going to clean up after her, but it helps a lot!
2. Baths with some baking soda sprinkled in.
3. Aquafor instead of creams
It sounds like she is allergic or intolerant to something else she is eating. Besides the skin, one of the other big detoxers of the body is our elimination. I am gluten intolerant, but I do cheat once in a while (and sometimes, I'm in situations where I have no other options, or I get glutened and don't know it). And I always pay with anal itching. So I know, without a doubt, that food can cause issues in that area.
It is probably time to take her to a naturopathic doctor and have her thoroughly tested for any and all food/chemical/perservatives/etc. allergies and/or intolerances. Like another poster stated; until you delete/take away what is causing the problem, she will continue to have issues down there. Take it from someone who knows.
There is something you can try. It is an all-natural/organic/no pharma cream that you can buy from www.aidance.com called femmesil. It is a cream that gets rid of bacteria, fungus, and yeast in the genital/anal area. It also will get rid of issues from food allergies/intolerances. I know, because I use it when I get glutened. The cream is gray (sort of like ambusol (?), the hemmrhoidal cream), and if there is something it needs to "clear up," it turns it brown (caution: she's in a diaper, so the diaper will get stained; if using it as an adult, you'll want to use a pantyliner to save your underwear). It works wonders. In fact, my daughter had a yeast infection and it cleared up within three days. All without any pharma or over-the-counter drugs. However, there is a caution here; while it will help clear up the infection/diaper rash she has now, it is not something that should be used forever. You will STILL need to discover why she is breaking out. And maybe just the fact that you've finally found a way to clear up the constipation (was she having diarrhea before?), combined with using this product to clear up the last of the irritation--that might be all she needs. But I would closely watch it.

My niece has cystic fibrosis (NOT saying yours does!) and she has to take an enzyme pill before meals. Something with her body's ability to digest food was causing the AWFUL painful rash's. Maybe do some research on this. Sorry can't remember the name of the enzyme. Maybe with the Celiac Disease she is experiencing the same kind of thing? Good luck...poor baby..ouch!
