Safety Plan for My Teenager

I need a safety plan for my teenage daughter. She has a bunch of safety concerns. I found one online last yr via google but I can't find it. I'm looking for something that I can use at school and home. She does a lot of unsafe behaviors and I need it in place to keep her safe. She is 13 yrs old and her therapist is currently out of town. I'd like to use a form of some sort to fill out that we can use at school so school knows what is going on at home and use at home for her PCA as well as me when she does unsafe behaviors to keep her safe. Any input would be helpful. I know I can make one but it would be helpful to have one that I can go off of atleast. Looking forward to your ideas :)

I'm a little confused:

Do you mean safety like what to do if she misses the bus, or there's an emergency, or latchkey hours, or drug use, or curfews, or....

I mean are these daily "chores" to check off (like wake up, dress, eat, catch bus, go to school, etc.)...

-or-

"House Rules" (like if you miss the bus you have to walk, HW before going to a friends house, call before x, be home by y, etc...)

-or-

"Unexpected Events" (illness, fire, power outtages, car problems, etc.)

-or-

"Consequences" related to House Rules (break curfew x happens, use drugs y happens, skip school z happens).

I'm assuming you mean mental health safety concerns. I searched and searched but I couldn't find an outline that can be printed, but I did find this:
What to include in a safety plan:

Warning signs of a suicidal crisis for that individual (such as increased depression or negative thinking)

Coping skills the patient can perform on his or her own

Family members and friends the patient can contact in an emergency

Therapist’s contact information

Phone numbers of emergency services available 24 hours daily (such as 911, suicide hotlines)

Since her behaviors are only her's it would be hard to come up with a generic form of any sort. I would certainly sit down with her and talk about unsafe behaviors and chart them out. Yes this is the hard way but probably the best way to do it. Also at this age, they are going to start trying behaviors that were not there before, they are starting to stretch their wings a bit. Let her grow but be safe also. Let her use her own judgement instead of ours, while yes she will have some down falls she will learn from these. We can not totally safe guard our kids, they have to learn some things the hard way. Good Luck

Mom. from what you say I think it sounds like your daughter may not use her best judgement always and you want a plan for her at school in case problems occcur, am I right? Good idea to be proactive and start now before school is in full session.FIrst I'd call and make an appt with her school guidance staff and ask they meet with you to go over your concerns for your daughter as soon as you can. If I am not right in what you are looking for I imagine that these people would have the expertise if you tell them what type of plan you want to put in place to steer you to the right peole to help you, but I would assuem they would be the experts in helping you create this plan combined with her therapist when he/she is back. Be sure you ask them to keep you fully informed of any problems or situations that occur at school with your daughter too.middle school can be tough socially and academically when they are preparing your child for high school. Good luck to your daughter this school yr!
