Doctor.

Parasites of many types
Malnutrition (kwashiorkor comes to mind)
Twisted Intestine
Partial blockage
Enlarged : liver, spleen, gallbladder...pick an organ, any organ
Lymphatic system
Certain tumors
Certain cancers
And about 66 other things....

OR completely normal

Only a doctor who can SEE him, examine him, and run lab years will be able to tell you whether its a minor, major, or no problem.

MY first thought is kwashiorkor. But that's because I've lived in areas where kids never get meat, and live off carbohydrates (casava, pasta, and potatoes, mostly). Without an exam and history, there's no way to tell what's wrong.

Definitely take him to the doctor. A friend of ours had a daughter about that age, skinny, didn't eat much but big belly. They found out she had some issue with her stomach and the muscles surrounding it that she'd had since birth, but didn't know.
Not to say that's what it is, but there could be a number of things. I'd just get it checked out.

I would try taking him to the doctor, just in case. Some boys (and men) do have that body type, where all the fat collects in the tummy, but if he's eating very little, that's a cause for concern. There are a number of medical conditions that cause weight loss with abdominal swelling. Most are treatable, but they need just that -- medical treatment.

It's definitely time to have him seen by a doctor. Right away! Please call and make an immediate appointment.

This doesn't sound normal. How long has this been going on? You need to take him to a doctor.

Start to make a list of the time of day and what he eats

I have a friend and her daughter would get stomach aches, have diahhrea and was really tiny with a big stomach. It turns out she has Celiac's Disease (gluten intolerance). Does your child get stomach aches or have intestinal pain? If so I would have that checked right away and in the mean time try taking all gluten out of your child's diet and see if it helps.
