I've used avent pacifiers since he's been born since he loves them. Now my son is a big boy 24 lbs so I give him the 18 month old ones (largest one they have) I couldn't believe it when he looked over at me smiling with the entire pacifier in his mouth. We went to Babies R Us and found Tommee Tippee pacifiers that were really long on both sides. I hope the same thing doesn't happen with this one too. Has this ever happened to anyone before?
Wow! No, haven't seen that before! Better watch that kid around the billiard table... I had a cousin who had to go to the emergency room because he put a pool cue ball in his mouth on a dare. I have no idea how they got it out but since then he has been the butt of the family's "big mouth" jokes (and it's been 25 years now... ;-)
Time to take it away, and keep a real close eye on this one.

I would definitely just wean him from the pacifier. It's not worth the risk! Even if he can't "swallow" it completely, it could still get stuck and suffocate him. GO THE SAFE ROUTE! GET RID OF IT TODAY!
Oh yeah. They can't swallow them... but they can get "stuck" (aka where they can't get them out on their own, but you can get them out very easily.
The upside to getting them stuck is one of two things happens:
a) They don't do that again (or at least not more than one or two more times)
or
b) They self wean from the paci out of disgust
