A dear friend of mine is getting married this weekend. Nothing big, family and closest friends, in the living room in front of the Christmas tree. She has a 4 year old son from a previous relationship. Her soon to be adores her son and is a great second dad to him.
As this is not just a joining of man and wife, but really the making of a family. So they want to incorporate him in the ceremony and maybe give him something too. But they are at a loss as to what or how exactly that would look. I have seen in weddings before where a daughter is given a necklace or bracelet after parents exchange rings, but what do you do for a little boy?
Any suggestions?

How about they each put an ornament on the tree and then the three of them place a special new star on the top?

We did a family pin (it had three interlocked circles...like the wedding ring symbol but three instead of one). It comes w/ a card with an nice little saying to be read about being a family.

This is the exact thing that happened to me, only I have a 7 year old daughter (5 at the time). We made her the flower girl and she was happy to do it. The little boy could be the ring bearer. If they need to give him something, give him a watch to symbolize all of the new times to come.

I had a friend give the little boy a ring as well, that he could wear when he was a little bigger, they put it on a chain for him to wear now. Also they did the lighting of the candle and he helped her light hers and then the one together.

My son walked me down the aisle and then stood between my husband to be and the best man. He was a junior best man.

Some great answers! I looked at ebay to see what is out there, and I found a cute little snowman family of three pin - lots of love on the snow family's faces and the little boy snowman has both parents' arms on him. Only thing is, it's a cheap piece of jewelry. Look up "High Polished Loving Snow Family Pin Pendant Jewelry" on ebay if you want to see it. Of course, he's four, so he might lose something more expensive.

a custom pin with interlocking initials (same thing in the inside of the wedding bands)...the little boy will enjoy carrying it on his jacket/shirt and it'll be a very emotional moment when mom and "dad" will pin it on him.
