Did you try the toddler sizes (2/3T, 4T) from the infants/toddler section? They have these at Target, Walmart, Kohls, etc.

Just wondering if anyone has found underwear that would fit a 20 lbs, 18 inch waist little girl. The basic brands in their smallest toddler size, 2T, (Gap, Gymboree, Old Navy and Target/Walmart) are all too big and sag off her. We do have some Potty Patty training pants that work well. Any other suggestions? Thank you!

Try googling underwear elimination communication. You might get some hits from companies that cater to the EC crowd. They want/need very small sizes for their little ones.
I went with Hanes. They sagged a little, but didn't fall off my child's waist. She was right aound 19 or 20lb.
My daughter potty trained at 2 years and 2 months... weighing abou 20 pounds..
I had no problems with size 2T/3T undies I bought at Target/walmart .. they had dora or some other character on it..
Gymboree runs big..
Stage and Gymboree, once you wash them in hot water they will shrink and should fit...we have the same problem

My daughter was petite, too. Hanna Andersson underpants worked best for us.
Walmarts shrink pretty good if you wash and dry them in very hot water.
Gymboree also carries XS - those are too small for my 2T gal so might well fit!
My daughter had a tiny body too. I think we got some from Kohl's that worked--I think it was maybe Hanes with My Little Pony and other characters (not my favorite decorations, but they worked!)
look in the infant section of those stores.
