Have you looked into an early intervention program at the local elementary school? Since you say that he has always had speech problems, I would think this is something you can't just ignore and the earlier you begin to have him seen by a speech therapist, the easier it is going to be for him to correct whatever issues he is dealing with.
My 2 1/2 year old grandson ( who does NOT have underlying speech problems so this is not the same situation) went through a stage of "stuttering"....he would start off each conversation by repeating the first word that he said over and over and OVER again!!! IT was like his brain was moving too quickly for his mouth to keep up. His parents chose to deal with it by ignoring it, after saying something to the effect of "slow down a bit babe so we can understand you"....it worked itself out in a month or so and doesn't seem to be an issue anymore at all.
Only a Speech Therapist can tell you if this is just a "phase" or something that requires some special attention.
You should take him to a speech therapist. The problem will likely become worse as he gets older, and it will be especially difficult for him once he starts school. My younger sister stuttered when she was young and I remember my mom taking her to weekly appointments with the therapist. She's 50 now and hasn't had any problems with her speech for over 40 years.
When he starts stuttering tell him to "stop and start over and slow down this time." He may needs some reminding again and again to slow down. My husband was a stutterer and does still every now and then. He had to work for a while to stop it.
See if slowing him down so his mouth can catch up with his brain.
My sister's oldest son didn't talk until he was three and also stuttered. They live in NC and she was able to take him to the speech therapist at the elementary school for free. He started when he was 3 or 4 and went once a week for maybe 2 years. He is 14 now and hasn't had any other speech problems since he finished. I would definitely try to go through the school system. I live in Memphis and my kids go to private school. My oldest who is 7 sees a therapist for his sh sounds. The therapists at the school system here are so overloaded that it takes several months to just get evaluated so we ended up paying a private speech therapist to see him and it is not cheap!
