Yes,Take her to the doctor. It could be not ant bites, but MRSA. She would need antibiotics. Also, I wouldn't use nystatin on it, as it doesn't sound like yeast, but use a triple antibiotic ointment.
Help! I noticed two nights ago that my 4 yr old daughter developed an "ant bite" looking sore on the top part of her vagina (above the lips). Last night, I noticed that she developed another and tonight in the bathtub I noticed yet another. What could it be? I applied Nystatin (she complains that it hurts when I do this) to them every night since I noticed them and they seem to have shrunken down to the size of a very small pimple, but yet more keep appearing. She doesn't have any discharge and the opening to the vagina doesn't seem to be redder than normal. She doesn't take bubble baths either. Has anyone else experienced this problem first hand or know what it could be? She is going to see the doctor on Wed. but I am so worried about what it could be. Any help would be greatly appreciated:)
I just want to thank everyone for their response. It's nice to know that if I have parental concerns that there are other moms out there willing to help another with issues like this! To put my mind at ease, I went ahead and brought her to her pediatrician this afternoon just to make sure that it wasn't serious. He diagnosed her as having Folliculitis (a staph infection that affects the hair follicles) and he said that because we caught it early that I just needed to apply Bactriban to the pustules after washing the area with antibacterial soap. And if that didn't help, he gave me a prescription for Bactrim to take orally. I was just glad that it was not MRSA.
Thank you again and God bless you and your families.

Glad to hear everything is working out. Sometimes you just need a doctor =)
my son started doing that (he is 2) but it did not look like a normal diaper rash, it was more on his thighs. 2 days later, daycare calls saying come pick him up we think he has chicken pox. I took him to childrens and they diagnosed him with hand foot and mouth disease and he had blisters on his hands and feet and the bumps in his diaper area. It is a virus that they are infectious for about 10 days to other children and you just treat the symptoms. hope this helps, look for blisters in between toes and on her wrists
Good luck at the doctor today. I'm wondering if this is the beginning of the measles. You may want to ask.

It could be a number of things...do they itch her?
Has she had chicken pox before? I know it is a strange place for them to start, and they are progressing slow for chicken pox, but if she has had the shot, that could be slowing them down. But, I think your best bet is just to get the docs opinion. Good Luck.
It could be nothing more than a heat rash.
Have you changed laundry products recently? If so, she may be allergic to your new detergent or fabric softener. Because underwear stays closer to the body than other clothes, contact sensitivity is sometimes greater around the genitalia.
My six year old daughter developed similar bumps 2 years ago on her bottom. At first, they looked like a little like a pimple, but without any fluid center. It was almost as if you could pop them, but you couldn't. When they started spreading all over her bottom, I became alarmed and took her to our doctor. He diagnosed it as molluscum contagiosum, a fungal-type of issue. He said she could've gotten it anywhere...pool...bathroom..virtually anywhere. He said there were tiny "bugs" inside each bump that wouldn't go away on thier own. So the 1st treatment he did was the chemical tincture that "burns" the bumps much like how warts are "burned." Then she had a recurrence bc we had missed a couple of the bumps and they spread again, so the last time, he chemically "froze" them off. The sores that developed after this treatment were awful until they healed. But she was bump-free after that. As far as I know, there is no over-the-counter treatment.
Sometimes yeast infections look like that. My son got one after a really bad diaper rash and the docter prescribed an ointment that took care of it. Also, he got the chicken pox and most of his spots were in his diaper area. He only had 3 or 4 spots on other parts of his body. If she sweats a lot down there her panties may not be dry and that can cause irritation and can cause bacteria to grow. Sometimes bladder infections can make little spots like that and she may not even show other symptoms of bladder infection. Your docter will probably test for this. The good news is all of these things are really easily treatable. Good luck and let us know what happens!
Im glad it didn't get worse.
