I have had my daughter since she was a baby.Her mother was never part of her life except for the first 8 months of her life.So it has just been the two of us and now that she has applied for the military we have taken notice that her birth certificate doesn't have my name on it just her mother ( the incubators ) name.Where is it that I need to go to have my name put on her birth certificate? I have no idea where the incubator is nor would I care if it hadn't been for this matter but any attempt to find her have led no where.This is really important to both my daughter and myself !.So can someone please help us.

The matter was easier to fix than I could have ever hoped for. Since I had already taken the ( incubator ) to court in the beginning month's of my daughters life to have her rights taken the paper trail was already there. So all the is needed is for me to wait for the application to arrive from the clerk send them back filled out and notarized with a few dollars in a money order and sit back and wait for her new birth certificate to be sent back to me...So again I say to all Thank You.

wait. I'm confused. You have an ADULT child and you are just now noticing that your name isn't on the birth certificate?
I'm with B on this one...you didn't notice in school enrollment? Drivers license...insurance...none of this before now?
Call your local family court (in some areas that's juvenile court, in others it's probate court) and ask the clerk's office for information on the process to amend a birth certificate. Have you been legally adjudicated as her father or have you been raising her without any court proceedings? If you had ever gone to court for custody or support activities, then at some point it's likely that you have already been legally recognized as her father. If that's the case, it should simply be a matter of submitting that documentation and a couple of forms to the state department of vital records for an amended certificate.
If you haven't dealt with court before and haven't been legally named her father by court, then you will probably have to do that first and then the judge can order an amended birth certificate. It's a bit of a pain to go through the process of filing the paperwork but nothing you need a lawyer for or something that should get in the way of you doing this.
ETA: The link that Wild Woman included is great, but without the mother available to sign the forms, most of these forms won't work for you. If you could track her down somewhere, getting her to sign the first form would be the fastest way to have this done. Assuming you can't find her, then you'll need to establish paternity via court first if you haven't done so already. Good luck!
Here in Michigan the father simply needs to sign an affidavit of parentage. You do not need mom(incubator) to do this. You can even call family court and ask them to send the documents to you.
I don't think you would need a lawyer unless another father was listed (not the case). Good luck-and congrats on your daugters new military career! You must be one proud single dad (or Mr. Mom)!

How about calling your local office of vital statistics? It will take a lot of time and money...it's not free. You will need to ask them for an Application for a New Birth Certificate Based on Parentage form. Make sure you have a list of questions to ask...

This whole process can take MONTHS....when does your daughter ship off to basic training???

You never noticed this all the times you needed her birth cert like for school enrollment, establishing ID for getting a drivers license, getting a passport, etc?
You might not think much of her mother, but she gave you your daughter (I assume you love your child).
It's just low class calling her an incubator - you're better than that (at least I hope you are).
If you (and your daughter) want to amend her birth cert, consult a lawyer and see if it can be done - it'll probably vary by state - do you still live in the state where your child was born? Is she over or under 18?

You might have to have a paternity test to prove you ARE her biological father (what if you find out you are not?).
I'm not sure the military cares one way or the other.

Othwerwise.... You are saying you have a copy of the birth certificate. How did you obtain it? I would think you would start there.
Or contact the courthouse where your daughter was born. I think if she is an adult wouldn't she be pursuing this?

Something's not right here. How did you enroll her in school? They DO read the birth certificates. And apparently she never went to a licensed daycare because they check as well. I had to have my GD's birth certificate and my letters of guardianship to put her in school and daycare. There is something missing here.
