I love our doc and the others at Mile High Family Medicine. We see Dr. Schindel. I've known him and his wife since high school. He's been great with our son with special health needs, too. ###-###-####
As far as writing the letter, I usually do that type of thing and then have my husband or mom read it over to soften it, but having worked in several similar situations, it is helpful to know what the issues are. It probably will go to the office manager. If it's a huge practice, it will go to personnel. Either way, someone will be taking note of the lost business, especially in this economy. The key is to by kind and calm. If you berate the doctor, it is more likely to be written off as you are the type who would be difficult to please. GL with both!
