My Two Year Old Screams When He Urinates!

My son has recently been very upset whenever he pees. It's hard to tell if it's every single time, but it's always painful when he is in his carseat or stroller. I took him to the pediatrican yesterday & they collected a urine sample but the initial test showed no sign of infection- but she sent it off to lab to test further for bacteria via culture. If he doesn't have an infection, then it will be somewhat of a mystery. She consulted with a urologist who suggested sitz baths & tylenol every 4 hours. We are waiting for the culture results before seeking further examination. Has anyone dealt with this before? Any ideas on what could be bothering my son? Also, he is circumscribed.

Your poor little guy!!

If the test comes back negative, I would ask for another test. I haven't had this experience myself, but I know two moms whose daughters were initially diagnosed with negative cultures, but the doctors did a more thorough test and they DID find a bladder infection after all.

If your doctor says no to the second (I believe more extensive?) test, then take him to another doctor who is willing to look at this more carefully.

Finally, is his private part (don't want to write the word or this email could be deleted!) red or irritated? Is it possible there is a tear or cut on the outside?

My daughter was playing at a park when she was about 2 years old and she fell down in some bark and branches. She cried initially and then cried off and on for about a week, particularly when she peed. When I was finally able to get a good luck down there (much harder to do with a girl!), I saw she had a small cut. Urine is acidic and it was no doubt stinging her everytime she peed. Once the cut healed, she stopped complaining.

A. - It's pretty rare for such young ones to get UTIs/bladder infections, but it does happen. I thought my daughter had one when she was two because she was having terrible, sudden urges to pee, and said it hurt when she did, but she was going so often there was barely a trickle when she went. I'm quite familiar with bladder infections, having dealt with them for years, so I took her in to our Ped. No infection was found, but I went through the motions of treating it anyway: Lots of water if he won't drink pure cranberry or blueberry juice (check the label; sugar or high fructose corn syrup will only feed an infection), yogurt sprinkled with acidophilus powder, and cut out any unnecessary sugar and white bread/pasta.
My daughter was feeling better within two days, but continue the treatment for about a week to be sure it doesn't come back. By the way, Tylenol isn't a very safe drug for children - I'd definitely treat the pain topically with calendula cream before giving aspirin or ibuprofen.

Take him to Dr. Guy Hudson - pediatric urologist who works out of Children's hospital and he also has an office in Bellevue. Great doctor. Great bedside manner.

Hi,
I had this with one of my daughters.. they collect a urine sample with a bag thing in her diaper at her dr. office.. it came back negitive for bladder infection.. they said nothing wrong.. but they would send it out to grow and double check it.. well 2 nights later she was still in so much pain and I knew something was wrong and I took her to childrens ER and they did a cathater (SP) and collect the urine a clean sample and sure enough she had a bladder infection.. It was the worst.. being sent home from the doctors and told she will be fine.. Childrens told me that it makes a difference whether it is a clean catch like they did.. and sure enough the dr. office called and said the sample they grew out was an infection too.. (gee thanks after I already been to childrens ER)

You might want to consider having him tested for urinary reflux. I had this as a kid and, untreated, it can damage the kidneys. It's propably nothing but always better safe than sorry.
