There are all kinds of things that cause allergic reactions, from foods to insects to plants like poison ivy.

I'd ask the pharmacist for an anti-itch cream like hydrocortisone, and I'd give the pediatrician a call to ask what else you should try (such as oral benadryl) or whether your child should be seen. It costs you nothing to make a phone call to the nurse advice line, it's more effective because the nurse or doctor can ask you relevant questions (and might even ask you to take a photo and email it to them), and it's so much faster than waiting for answers on the internet!

And today is Friday, so get that call in now before you're dealing with a doctor's weekend schedule.

Sounds like contact dermatitis to me - has he come into contact with anything that would irritate his skin?

Take him to the doctor. It could be molluscum contagiosum. My daughter got this a couple years ago and it took forever to get rid of them. They itch. They spread. Since you say they are rough they could be warts. I advise also going to see a dermatologist. It could be many things.

Because it is itchy and hurts, he needs to be seen by his doctor or a dermatologist. There are too many things that it could be to know without an exam.

What does your pediatrician say?
Rashes can be due to any number of things.
poison ivy
allergies
infection
etc
coming into contact with something that's burning/irritating his skin
A doctor will help figure out what's going on and will be best able to help you treat it.
