First of all thanks to all who replied and shared their ideas and thoughts.
The entertainment Web site offers the book for $5 and free shipping if you buy the 2012 book. This option would leave me with two books for 2012 because I usually buy them from my kids' school. So, the hunt is still on for a 2011 book with no strings attached :)

I ordered one two weeks ago using Ebates - www.ebates.com. Search for Entertainment.com in the store search field and it will take you to the website for ordering a Passbook. Today they have a special. Reserve a copy of the 2012 passbook and they'll ship you a 2011 one for free.
There are great deals on them online right now.

I bought mine off their website. They run great deals on a regular basis. I have also seen them at CVS.
I saw them on sale at the CVS on the Corner of Coit and 75/635 for around 30.00.
