Take him to the doctor as soon as you can. I would not try to guess at something like this on a young child.
My 4 1/2 year old son has something that looks like a yeast infection on his anus. It's red and itchy and has a couple small yellowish "grains" just like the yeast you bake with. I treated it a couple weeks ago with monistat external cream and it went away by the time the doctor saw it. It's back. My questions: is it yeast infection? How do I treat it? Thank you.

See your pediatrician again and ask him/her to culture it. We had similar bumps on our daughters back side (cheeks, not the anus). Started with creams, had results, got worse, got an antibiotic (oral) got better, came back. Through a culture, the doctor found out from the lab that it was a MERSA strand, resistant to all but two antibiotics tested. It was very difficult to get rid of, and once we knew which antibiotic would work it took more than one course of it to eliminate it. Apparantly MERSA like this is being seen a lot by pediatricians this year on young children, showing up in different places, and it nitially looks kind of like a diaper rash, but evolves into something that looks more like a pimple with a little white head or pus on top of some of the larger bumps. It also spreads very easily among children.

My son had something similar, it turned out to be strep. They did a swab around the outside of the anus, nothing internal, it was positive. After a round of antibiotics, it never came back. Apparently, he had a small tear from constipation, and allowed the bacteria to enter. Good luck-
if it turns out to be a yeast infection on his anal area, after you have it checked out by your dr....cut out sugars and sweets, because yeast feeds heavily on sugars. how is his digestion? probiotics are important in the reduction of yeast in the body...or candida is another term for yeast in the body. the probiotics are healthy bacteria that eat the bad bacteria (candida/yeast) in the intestinal flora and this will help the rash from the inside-->out, which topical medicated creams will not necessarily help...especially if it is due to his diet. here are some sites on probiotics...

Nobody reading this can tell you for sure what it is. If it's back, take your son to the pediatrician for an opinion.
My little cousin had this problem and it kept coming back he had something called canadiasis (spelling ??) and had to be on a special diet. I just remeber that poor kid always had an itchy butt until somebody figured out what was wrong with him. This probably isn't the case with your son but it sounded really familiar.
Go to the dr this time when it is active. He will probably give you something like bactiban or something like that.

It might be something he is eating or drinking that is causing it.
Even the toilet paper might be doing it. I would use pure white no colors.

Sorry, but this is "Gross out" time:
Take some Scotch tape, lay it down on the rash and pick up some of the "grains". Put them in a plastic bag. See if they move. If they do, he may have worms and the doctor needs to treat them.
Having said that and after rereading your question, I think that you should take him back to the doctor without trying to treat him again. Let the doctor see the rash exactly the way it is now.
There are all sorts of things that you can do if it is yeast. But you are already saying that it returned after treating it for yeast once ... The doctor really needs to see it as soon as possible, before it hides again.
