I am celebrating my kid birthday in his preschool and giving a gift bag that includes story books ,stationary, chocolates as a" thank you" to his class room.
What note or message am I supposed to write on story books ? Should I mention teachers name and the occasion on it?

when you say your celebrating your kids birthday does this mean your sending cupcakes and then a bag with books ? I am confused. I have never ever seen a public preschool or daycare center that allows an actual party to be held for a child in the center. but on to your question.
if your donating the books to the class room then I would not put anything other than "this book donated by... and your sons name.
I would write something like "This book was donated to Mrs. Smith's preschool class by Johnny Doe on his fourth birthday, October 10, 2013."
Giving books is always a good thing!
If there are only a few, you can write in them something like, "Especially for Mrs. X's class, from Bobby on his birthday.'' Keep it simple.
If there are more, or these are going to be staying in the classroom and not going home with kids (I'm not really clear), you can print out wonderful bookplates from free templates on the web and can fill them in on your computer. Search under something like "free bookplate templates." There should be ones suitable for kids' books, maybe with cute borders. I did this a while back when a class donated a heap of books to the school library to honor a teacher; each book got a bookplate (you stick it inside the cover or on the first blank page) along the lines of "In honor of Mrs. X on the occasion of her Y years as a teacher, from her fourth grade class of 2011-2012." You get the idea.
I'm with Jill.... Write a separate note, and give the book(s?) away unscathed.
On another note, I don't mind the note inside the book at all. I love reading the little passages people leave behind in a book cover when I pick up a used book.
