We put a standard pillow under the fitted sheet so he has the elevation and extra comfort... and we have no worries about him getting under it.
Can anyone tell me when they started introducing a pillow to their child? Any recommendations on type or brand that are child-size friendly would also be appreciated. Thanks!

I think I started when my daughter was about 12-18 months. Didn't bother with "child-friendly" or anything like that -- just threw one in there with her when she wanted to take one of the couch pillows to bed one evening.
She was definitely able to grab the pillow and move it around how she liked (on the couch) so I figured, "Why not?"
I got a small, child-sized pillow at Linens N Things several years ago, but I've also seen similar ones at Pottery Barn Kids. I gave them the small pillow when they started in their toddler beds (at about 18 months for my kids), and standard sized pillows when they moved to their twin beds (at about 3 or 4 years old). But honestly, they kept wiggling down off the pillow anyway, so it didn't seem very necessary. My son (6) has been using a pillow properly now for a couple years, but my daughter (5) still rolls all over the bed and is rarely actually on her pillow.
I'm sure your son is old enough to have a pillow, but don't feel bad if he doesn't end up using it for sleeping. It may end up as a plaything. :)
I just waited until my son asked for a pillow. I think he was about 22 months. He sleeps with an adult sized down pillow and seems to like it.
When my daughter was 14 or 15 months she grabbed my pillow one night and took it to bed with her. Standard pillow and pillowcase. She has used that one ever since. She will be 2 on July 28th.
