My son is 3 1/2 months old and he just started spitting up in the last week. He has never spit up until now. He only gets breastmilk. Does anyone know what might cause him to start doing this?

I would say he probably has reflux, try getting some zantac and see if that helps. Lots of babies/people are having it these days. Hope he gets better!
my son has acid reflux....try checking that out...he was born early which is common for preemie babies!! He is on Zantac 2xday. it has helped A LOT! he was also breast fed so i know it is somehting that can happen even if your child is BF.
When my son was almost 3 months he began spitting up A LOT! With Acid Reflux being such a big deal and so common right now, I thought he may have it and even considered taking him to the dr. I really thought hard about it though. He was very strong, very happy, made great eye contact, smiled a lot, didn't seem to be in pain or hurt when spitting up, etc. He spit up a lot shortly after he nursed (never had formula). He eventually got over it and I never took him to the doctor and didn't have him put on meds. I kept him still and upright for 20-30 minutes after each nursing. We went through tons of burp rags and clothes but we got through it! He's super healthy, happy and started walking very, very early! Good luck!
2 things come to mind; keep track of what you're eating to see if you can isolate what's irritating him, or because he's probably becoming more physically active, he may be spending more time on his tummy, etc. and that will cause some spit-up. As long as he's not crying with it, and he's not spitting up an entire meal, he'll be fine.
has you diet changed? he could be reacting to something you've been eating lately. an allergic reaction possibly.
Has there been anything different in your diet by chance? I know when I breastfed my son, I couldn't eat anything spicey because it upset his poor little tummy.
While he may be spitting up from reflux, he only needs medicine if he is uncomfortable. The common meds like zantac and prevacid only neutralize acid, they don't stop the actual spitting up. My son was on Prevacid for 6 months. It made him not hurt, but it didn't stop the milk from rising in his throat... the only thing that helped that was to thicken his milk.
have you changed your diet in any way? is the spit-up consistent (like only in the mornings or evenings or after every feeding).the reason I ask is that you might be eating or drinking something that is to much for the baby's system to handle (caffine increase ,not enough water ,spicy food ,ect.) I suggest you take a close look at your diet.
Didn't read other posts, so sorry if I'm repeating what other moms said. Could it be something in your diet? Maybe try eliminating all dairy (make sure you read food labels as milk/whey/caesin) is in LOTS of foods) and see if that helps. You might want to discuss your concerns with your pedi as well. My son battled reflux for the first 12 months of his life and had trouble gaining weight, but it was obvious that he was in pain and showed signs of discomfort while nursing (arching back, straightening out legs, crying, etc.)

Anything new in your diet, that he may be reacting to is a possiblity. I was also very careful to never lay my dd down flat after a feeding, babies should have their heads elevated in order to help with digestion. The flap at the top of the stomach is one of the last things to fully develop and keeping them upright always while digesting is very helpful. We had a lactation consultant who came to the house right after birth and she advised us on this and my dd has never spit up. I hope this helps, although this is a strange age for this kinda thing to start up at, maybe it is food you are eating?
My 30 month started spitting up not to long ago and it turned out that he was getting to much milk and not enough of solid foods. Once I reduced the amount of milk he was taking. He started eating better and stopped spitting up.
