I found with my Playtex system that medium flow nipples were really hard to find. They also have a variable flow that is supposed to more readily adapt to the speed the infant tries to maintain. Maybe that would help?
What is inbetween a slow flow and rapid flow nipple?

It sounds like you are using the Playtex products, and unfortunately there is no inbetween. If you want to try it you can heat a needle and put it through the hole in the slow nipple to make it a little larger. Otherwise you just have to wait until your little one can handle the rapid flow.
Platex has a tri-cut nipple that only flows when compressed as opposed to their slow or fast flow nipples. This way, baby (once he figures it out) can control the flow to fit his needs. I like this nipple because it does not drip if the bottle is tipped over or when you are guiding bottle in and out of baby's mouth.
The nipples should have tear drop symbols on the rim. Stage 1 is 1 tear drop, stage 2 the next. Stage 3 is the fast one. Look for a stage 2.
depends on the brand. Some have 1, 2, 3 (with 2 being the in between) and some have slow, medium, fast.
It was the nipple flow that caused my daughter to NOT drink from a bottle. Most bottles come with a size 1 (unless they label it zero) nipple, or slow flow. My little girl became frustrated as she would have to suck too hard to get any milk (she is used to my geiser of a chest, apparently). So, I tried a #3 fast flow. Worked like a charm, but even with a bib, wet shirt. So, back to #2 medium flow. Just right! lol
