Moving to the area this Wednesday and was informed by school that I need to have a Florida Dr. fill out my daughters updated immunizations/physical onto a blue form.

I take my 2 nieces who are mine now; and I took my son when he was a youngster to Pediatric Assc. Of Coral Springs! They are open late & weekends also! Remember to bring all of your daughter's medical records with her so that she will not be required to get the immunizations again! The FL doctor won't be able to complete the blue form without her previous records. It is easiest to get the records from your doctor's office where you are now yourself and hand carry them down to the FL doctor of your choice.
I learned the hard way moving from VA to FL - thought you might benefit from my lesson:)

I absolutely love Children's Medical Association. We use Dr. Penson primarily, but all of the doctors and nurses are fantastic. They are open 365 days a year and have three offices.
