Yes, you should.

Talk to your pediatrician about having her evaluated by an occupational therapist. She very well may have a problem with sensory integration. It manifests itself in a lot of different ways. An OT who specializes in sensory integration disorder can help your daughter.

My son went through this. He has scars on his lips from it. He had other issues too and his OT really did help. Early intervention is the key to making a big difference and fixing a lot of problems that crop up as she gets older.

Seeing as she's chewing her toes and lips, yes it sounds like there's some sort of problem.
Talk to your doctor about it and see what he says.

Yes, a 3 year old child that doesn't feel pain enough to stop biting their lips is a great cause for concern. Please take your child to their doctor and ask for a referral to have your child evaluated for mental health issues. This could be anxiety, stress, or a symptom of something else.

You've put up two very brief questions with practically no information. I think you might have combined them into one to give responders more detail. How long has this been going on with the lip? Did it happen once? Has it happened many times over a period of weeks?

But yes, if you have a child who is chewing on her toenail to the point of removing the entire nail, and if she is persistent about biting her lip (rather than biting it once and then perhaps finding the healing area to be something she fiddles with), then your first call should have been to the pediatrician and not to an anonymous internet forum. That toe should be looked at to be sure there's no infection going on, and the 2 behaviors should be reported to the doctor who may have additional questions about other behaviors you haven't mentioned or noticed.
