I have a Mirena (I live in England) and love it! I have had it for 3 years (after my second child was born) and I've hardly noticed it. The insertion process is uncomfortable, but not painful - a bit like a clumsy smear - and then you just forget about it. I think they check it every 5 years or something like that. I don't have a period at all any more, which is fantastic! I do have occasional spotting, which is a bit annoying because it's not on a schedule like your period. It just kind of sneaks up on me. My neighbour, who is a GP and runs sexual health clinics inserting Mirenas also has one and she doesn't get her period at all, no spotting nothing.
Also, if you do decide to have another baby, you can have it removed and get pregnant again. Mine are 3 and 5. I'm done, but 39 is not too old to have another so if you decided you wanted to it would still be a possibility with the Mirena.
