I have two different recipes I use:

Fresh horseradish and ketchup, (1-2 tablespoons horseradish per 1 cup ketchup, more or less depending on your taste - we like a lot!)

Or,

1 cup ketchup
2 tablespoons horseradish
1 tablespoon lemon juice
1/2 teaspoon worcestershire sauce
a little sea salt and a little pepper
chill and serve!

I know you asked for a homemade recipe. You've gotten some good replies and I'm sure you could find many online. I just wanted to chime in with my recommendation for the best store bought brand. I worked at a retail fish market for 7 years and I have tried all kinds. Make sure to serve your sauce (homemade or store bought) cold.

IMO, the best store bought one is Crosse and Blackwell with a green label. I recently had an opportunity to taste other brands and it reconfirmed for me that C & B is by far the best.

The fish market I worked at also made their own. The base was chili sauce doctored with horseradish and a few other things I can't recall.

BIL's family always makes their own cocktail sauce - even at restaurants.

They just sort of mix everything until it tastes right to them - they can do it in any quantity they need. It uses the same ingredients as Scarlett's, but no worcestershire.

I just eyeball the measurements, so feel free to adjust to your taste (this is very approximate).

1/2 c. ketchup
1 tsp horseradish
2 tsp Worcestershire
1 tsp honey
1 tsp white wine vinegar

Mix and chill. Yum!

Ina Garten has one on food network.com. Sorry, no time to look it up this moment. But it's good, and easy!
