Yes, I believe that it is start to start. It would count as 4 hr in your example.
How do you count the time between feedings? Do you count when they BEGAN to drink the milk, or when they FINISH the milk?
For example, if little one began drinking his bottle at 10 pm, and then asked for it again at 2 am, I would count it as 4 hours in between, like start-to-start.

I count from the time I begin a feeding. My daughter eats every 3 hours. She woke up at 6:30 this morning, I gave her a bottle right away and again at 9:30. Of course if she is hungry before that I don't wait until it's been exactly 3 hours.
I have heard that you do count from start to start, but I'm not sure what the reason would be to track this?
I usually paid more attention to how long or how much my babies ate. I'd glance at the time when they were done. Neither one of my kids ate every two hours or anything specific. I just paid more attention to how much went in them I guess.
With nursing you count from start time to start time. Not sure if it's different with formula babies.
Why would you count? I always just nursed my babies when they were hungry, before they would even start to cry, but just root around, and they gained weight on track and turned out just fine.
