I would try cotton undies between her and the pull up, and by all means a bath EVERY night. And I'd take her to the doctor.
My 5-year-old daughter has had on-again-off-again pain in her "t-t" in the middle of the night only after she's peed in her pull-up. (Yes, she still wears them to bed as she doesn't wake-up.) I've had her urine tested for a urinary infection and there isn't one. She wake me up screaming/crying hysterically. I generally have to put her in a warm tub, dry it off and put either a homeopathic vaginal cream or anti-itch yeast cream on her to calm her down. I don't like traditional medicine (ie: antibiotics, especially if it's not showing an infection.) but I do need to figure this thing out. One school of thought is that since it comes in phases (1 or 2 weeks then nothing for a couple weeks) that it might be the brand pull-up I'm buying (generic for the most part since they're so costly.)The other is that it's emotional and its manifesting this weird pain.
Any ideas at all would help. I'm a single mom and it's really rough going through this alone in the middle of the night...night-after-night.

I would look at her diet and if it's high in carbs, it could be a yeast infection - if she also has bad breath or white stuff in her mouth or on her tongue, it may be systemic. There are good herbs, although, you may need to go on antifungals for a while - abx will only make it worse. And, probiotics are a MUST in these cases. There are teeny tiny little ones that she may be able to swallow - Sprouts also has chewable ones.
Also, dairy can really aggravate the urinary tract and is a far more common root cause for bedwetting than most docs realize.

Have you tried wiping her before going to bed? I still wipe my 6 year old daughter a couple of times a week just to make sure there is no buildup... it might be TMI but some types of toilet paper leave a lot of debris when wiping and that's when bacteria multiplies.
have you noticed if it happens when days that she's been too busy to go potty? if she holds it too long, the next time it may be painful.
update: also, bath soap might be irritating her too.

i use melaleuca products on my girls (the whole family uses melaleuca body products.) here are a few suggestions i noticed have not been mentioned. have her take a shower either alone or with you. the reason for this is the shampoo and conditioner you use in her hair. many shampoos and conitioners have the preservative Quaternium (formaldahyde) in them. quaternium is a known carcinogen and it is also very irritating to the vaginal area. the goal is to not shampoo& condition her hair in the same water because it irritates her vaginal area and causes yeast infections. also feed her yogurt if she likes it and that will help with keeping the yeast under control. at this age they are geared up playing and when they go to the bathroom they are in such a hurry to get back to playing or whatever they forget to wipe.
please start checking your labels on your shampoos, conditioners, lotions, etc...you would be surprised to find out what chemicals are in our body products.

Well my first thought was UTI, so I'm not sure. I know that when I was younger my mom said that my urine would smell like amonia, and sometimes I notice that still, but not very often. When that happens, it's almost like my urine is hot and burns a little (TMI I know). I don't know what that is or ever had it checked, but it does happen every so often. I wish I could help more, but it may be something like that. Does it have a strange odor? Also, you may want to just try a different brand of pull up and check the fluids she's taking in. Make sure she's drinking plenty of water. You may even want to just place her on water for a day or so (with food of course), and see if there's a change.
does she get a bath every night? if not then i would start bathing every night because they don't always wipe well & even if they do a lot of toilet papers are irritating.
Hi S., my grandaughter did the same thing for a while and we took soda out for several weeks. By adding them back in a little at a time we discovered she reacted to rootbeer which she loves. After cutting that for a year or more she can drink it once in awhile but too many and her burning and itching is back and she complains that it hurts. I don't know what your little girl drinks but try the process of elemination. Then add back a little at a time. She may also need to see a urologist. Her ureter ( incase you don't know the tube from the bladder out) may be too small and need to be dialated. that's not uncommon in little girls.Hope she is better soon. Children don't usually fake pain in the middle of the night so do all you can to get her some relief. I had kidney stones as a child and they aren't easy to deal with.Take care. Have you had her tested for a yeast infection? My niece used to get them bad when she was little. The doctor can prescribe some stuff, and you can start giving her yogurt daily to prevent future problems.
Could it be that she's just doing that to get attention or using it as an excuse because she doesn't want to admit that she had a bad dream or that something in her room or a noise outside scared her? Have you tried teaching her to change her own pullup at night and to wipe the area with a wet wipe? If it doesn't happen consistently when she urinates at night and you don't see any visible infection or redness/irritation, I would think it's psychological. Next time it happens, make a note of what she ate the day before, as well as anything else that happened during the day. Did she go to school or no? Did she have more or less time than usual with you? What did she watch on TV? What was the weather like and did she play outside at all? You may eventually notice a pattern.
If she doesn't have kidney infection, sounds like it could be too much acid in her system, so when she wets it would burn. It could also be the brand of pull ups, and some detergents, and espically softners make some people alergic. Sounds like she is raw there and so being wet could burn. Or she could have yeast infection. Has she been on meds for an infection of some other kind, as this will give it to people of any age. Just a thought.
Instead of pullups why not get the girl overnight pants from www.pottymd.com or www.bedwettingstore.com. That may be better.

Probiotics would be good.. it sounds like something she is eating is making her urine acidic. Generally if this is happening she is on the yeasty acidic side. So is my 3 year old daughter and I. What I drink before bed is 1/2 - 1 teaspoon of Arm & Hammer Baking Soda is warm water. For my daughter who is picky and would never take it if she tasted it, I put like an 8th teaspoon in everything she drinks in a day i.e juice, milk .. I do this during times of acid and yeast. I would also bath your daughter during these episodes with 1/2 - 1 cup of baking soda in a swallow bath. When the 1-2 weeks in on, always sit her in this before she goes to bed - even if it can only be a 5 minute splash. You should have a pretty quick reaction if this is it and getting better with time
I like to keep things natural too, but I think it is time to have a doctor take a look.
