I know you don't want harsh, but the only thing I've found to work is a product called The Works. I have never had it fail. I usually avoid cleaning products as well but sometimes exceptions must be made.
My friend just inherited her mom's house and we are trying to get it cleaned up to sell. It wasn't kept up well and is pretty run down although the house is only 10 yrs old. The main bathroom is awful...we are going to have to replace all the tile in the shower due to extreme mildew. We were able to scrub the grime away from all the hardware and faucets. And we got the toilet cleaned up and may not have to replace it IFwe can get rid of the waterstains under the rim. We have used everything from Ajax, to soft scrub, to vinegar and even a bottle of soda! We don't want to use anything harsh but if you can suggest something that will do the trick, we are willing to try anything at this point.

We have very hard water here. The house we bought was empty for some time, so it developed a ring hard water stains. (Where the water line was.) The ONLY thing that worked for us, was a pumice stick. We bought it at Lowes. That thing was incredible! Here is a link for what it looks like:

Go to the grocery store and look for a product called Wink --- not Whink-- it comes in a brown bottle and gets rid of rust stains.
Also go to Family Dollar and get a product called Mildew and Soap Scum Remover ($1). I have white sinks and my kitchen sink gets stained --- a few squirts and I leave it soak for a few minutes, rinse and my sink is white again.

Use the stuff from Family Dollar on the ceramic tile and get it as clean as possible. I wouldn't replace it. The plan is to sell the house. I would not like it if I were purchasing a home and it was all updated with new carpet and tile and paint. I would rather buy a fixer upper and do the work myself. Unless you go with all neutral colors which is visually boring --- your taste may not be the buyers taste and they will do it all over again anyway.
There is a pumice stone you can get at the hardware store that will take those stains out. It is safe for the porcelain (its made for it) and you will have to scrub..
As for the tile, consider having it re-grouted. They will scrape out all the grout, dry it all out, then put in new grout.
Good luck, and sorry for all the mess.
it might not be a stain. It might be scratched. Is it gray in color? That might mean scratch.
Good ol' Lysol toilet bowl cleaner does the job as long as you use a brush to swish it around...Depending on how bad the stains are you may need to repeat a number of times
I sprayed mildew remover all over the bathroom in my old renthouse and the mildew and grime wiped off easily. You can also try CLR.
Lysol black label toilet cleaner with lime and rust remover......let it sit and sit, scrub, rinse, reapply & let it sit some more!
The only thing that works on the hard water in houston is something to remove the scaling. Go to a plumbing store, they have stuff. It is pretty powerful. It will take the scaling off the toilet and leave it looking shiny.
I cannot remember the name of it, just ask them.
Good luck!

I second "The Works." I tried everything to get the yellowish, off-color off of the toilets when we moved into this country home. And imagine my surprise when I poured it onto the stains and the stains disappeared without me doing anything. It was immediate! And the best part is that The Works is only a dollar at Dollar Tree! They probably have it at Walmart - I just never checked.
I used and still use the pumice stone as well.... It works GREAT.. I bought them at Lowe's. Good Luck!!
Use "The Works" Its cheap and it really works on rust and water stains. I would wear a mask though because it is pretty strong. Also you really don't have to scrub.
vinegar should have done the trick. perhaps it did not soak long enough. i would wad some toliet paper up and stick it under the rim soaking for a few hours see if that works.
i believe its called c r l in a green squirt bottle its made for getting the hard water, lime scale and rust ?? i think .
I used Comet/Ajax but with one of those silver metal scrubbing pad things you use on pots. It was shinny white afterwards. Good luck!
