Being brought up Catholic myself and having had to attend these, I have found that money is the gift that is given most often. $20-25 is a nice gift unless you would like to give more.

A small monetary gift in a First Communion card is appropriate.
Yes! Take a gift. Find a chrisitan book store, they have great faith based gifts. Maybe a children's bible, a St. Christopher medal, rosary beads etc. They will have a section for children's gifts. This is a big milestone in the spiritual journey of this young man, and its a big deal to us Catholics. Good luck and congrats to the little man.
You should definitely bring some sort of gift. We have been to a few, and anything religious is always appropriate. (A new Bible, a religious book or movie, etc.) I suggest googling "Christian book store Thousand Oaks" to see what you can find. They have SO many wonderful things there, and they may even be able to come up with something creative.

Jewelry would be another gift idea, for example Patron Saint charms on a bracelet or pendants too. For a girl, tiny cross earrings in gold or silver or a delicate link bracelet with a cross charm.
I brought a nice photo album, it was actually made by a neighnor, but it was really nice.
Money is usually what is given for a Communion. You can go to a Catholic book store and find appropriate gifts (photo album, medals, etc.) as well. My sister gave my son a wooden frame that was engraved with his name, occasion and date which was really nice.

Yes, a card and a small gift from a catholic book store - call the nearest parish church office and they can tell you where the nearest one is. They will have little items like prayer books, or a rosary, and appropirate cards. Ask one of the people working there to help you, they have helped many a non catholic select something perfect!
