You know, when something odd like that pops up and seems persistent, it's always a good idea to have it looked at by the doc.
You might have to avoid water, sand, sea water for a bit if you want it gone quickly.

We were fortunate never to have any diaper rash when DS was in diapers. Now that he's in underpants, save for naps and at night, he's developed a rash along his inner thigh. It looked inflamed, then spotty, then had whitish/ yellow heads, then seemed weepy, and might be on the mend. It doesn't seem to be responding to desitin. Its about a half inch wide, and maybe an inch and a half long; and he's had it for about 2 weeks now.
potty training, sweat, summer heat, water play, sand, sea water etc are probably not helping.

Yep, probably yeast infection...especially given time of year like you said with heat and moisture. Try some lotrimin but if that doesn't work then see doc. Our kids got them a lot and we would use a blow dryer on cool setting to help dry them off while they were in diapers to avoid more moisture.
That doesn't sound like diaper rash to me. Sounds like an infection. I'd take him to the doctor.
Sounds like impetigo. Instead of diaper cream, use antibacterial cream and keep dry. If it doesn't clear up completely, see your ped. You might need something stronger.
Doesn't sound diaper rash-y. The white/yellow heads and the weepy part sound a little more like an infection. If it was just weepy, with blisters, I'd say something along the lines of poison ivy, which responds to an astringent like Domeboro. But the heads don't sound like poison ivy, and also it's not on other parts of the body. Due to its location and limited area, it sounds more related to moisture and lack of circulation, so I'd say covering it with diaper cream could even make it worse. Yeast is a strong possibility as others have suggested. You could try an antifungal, but then you still have to wait to see if it works. I think maybe I'd have it looked at, given that it's 2 weeks and it's not responding. Our pediatrician told us not to use any powder (ever) because of yeast. If you don't want to go in, at least call in to the pediatrician and discuss it with the nurse.
It could be a yeast infection or jock itch.
Try some Tinactin cream or ointment on it.
Add 1 cup of plain yogurt to his bath water.
Some Epsom salts in his bath water would be soothing, too.
Make sure he stays very well hydrated.
If he's playing by the ocean and he pees and concentrated urine runs down his leg - it can burn his skin.
So always have him drinking plenty of water to keep his urine very diluted.
That sounds like a yeast infection to me, but it really needs to be seen by the pediatrician for a proper diagnosis.
He'll likely prescribe Lotriman 1% cream to put on it for at least a week. You can mix that with Phillip's milk of magnesia (plain) to help soothe the rash and minimize the acidity and blend all of that into a paste with Balmex diaper cream and it's a really great salve.
That sounds like an infection (impetigo, maybe) or yeast. Better get him to the doctor, for the correct cream or antibiotic. Good luck!
That sounds like a yeast infection or some other sort of rash. He needs to make sure it's not an insect bite too.
I use Dr. Smith's ointment. It's awesome. I can't find it in the stores so I order it from Amazon.
I am not a doctor! But, Yeast infection it sounds like, not necessarily diaper rash. would try cleaning it gently with baking soda and soap, then applying neem oil, and lightly bandaging. Or other ideas? Mich
