5 Year Old with Painful Urination

On Sunday my step sons mom told us jr had been complaining that it hurts when he urinates. Our first thought was either a cut or a uti. So we made a dr appt but none of us could take him because she and I had work and dad didn't have the car ( I leave before he even gets dropped off on mon mornings) so her brothers gf took him and dropped him off with us after. I'm confused about the dr visit. He said he couldn't have orange juice anymore and needs a stool sample. Jr said he peed in a cup, so I'm assuming that was for lab testing for a uti. We haven't heard anything yet. According to the gf, the doctor gave no diagnosis and just did the no oj thing (which i dont get and if someone could explain that i would appreciate it) Jr hasn't pooped here yet to obtain the stool sample. He apparently pooped twice at school and didn't have to go again today. Is this something we can ask the nurse to do? Or do we just need to wait it out. He is still in pain and its not during the stream, its when he stops. And he will stop, go a little again, stop, go again. I have had utis before and know they can be really painful. My doctors have always given me antibiotics even before getting lab results just in case. Should the dr have given him some? Does anyone know what else it could be? It's so hard to see him in pain like this and his 6th birthday is this Saturday and I feel so bad that it might still be going on by then. He's starting to be reluctant to pee because of the pain it causes. If it is a uti, how is this possible? And if its not,has this happened to anyone else's child that they know what else to do? I have azo tablets to relieve symptoms but i don't think kids can take them. Sorry I have a million and one questions but I'm worried the dr didn't treat him correctly. He is covered by Medicaid and can only go to this doctor. Any advice would be very much appreciated.

** sorry I should have been more clear that I won't be giving him anything without asking the dr first. I have been trying to get in touch with mom for hours and getting no response. I am going to tell dad when he gets home from work to call the dr tomorrow and ask and find out what the heck is going on. We need answers besides dont drink oj and get a stool sample. I am just trying to see if any other moms have gone through this in the meantime. And as far as boys and uti symptoms, that's actually exactly what I said Sunday night. I read a little and it said bladder infections rarely show symptoms but other parts of the uti track might. And rare as it might be, I'm a little worried about kidney stones because he's saying the same thing my step dad did when he had them. I really feel like the dr should have done more. We waited to see if it got better today, but dads been at work since before jr got off the bus this afternoon. I can't call because the dr def wouldn't be allowed to give me any answers, hence why I've been trying to talk to mom. But like I said, no response.

**in response to the er, our local one is horrible. Worse than the doctor. They almost killed me by misdiagnosing me for over a month when I had an ectopic pregnancy that eventually ruptured because they never found it in the 9 visits i made there. They are quick to dismiss people and have never once helped him, me or anyone when we've taken him there. The best one to go to is about an hour away and dad is still at work until midnight. If we need to take him to the far one we will but I want to wait to hear back from his doctor first. For all I know his labtest will be back by tomorrow and we can get medicine called into the pharmacy. It's just really hard to do anything because we have one car, mom barely will take off of work for him, and I am already on the brink of being fired for the amount of times I've had to call out for things involving taking care of him. Dad doesn't have a license right now either. So it all kind of falls on me.

So What Happened?

Mom finally got back to me. Urinalysis said no uti. He thinks he either holds his pee too long or is constipated. Neither of these sound right to me either since he poops frequently and healthily. And he never pee pee dances or waits until its urgent. He said for us to have him pee every 2-3 hours but he does this anyway. Even at night, where he still doesn't wake up naturally to pee, his father or I wake him up every 3 hours to go so he won't pee the bed. I think he still wears pull ups at moms. She also didn't bathe him all weekend so he went from wed night when I gave him a shower til Monday night when I gave him another one. Not sure if that has anything to do with it. Like if there is a hygiene issue. We don't do baths anymore because his skin dries out and he gets itchy. Mom still gives bubble baths against the doctor recommendations, but most of the time he comes home and says he didn't get anything at all. I don't know. Even that doesnt seem like a likely reason why it would hurt him so much when he stops peeing. None of this makes any sense to me. I just feel so bad for him that he has to endure this kind of pain. I think his dr is a quack.

Are you able to call and speak with the doctor to clarify his recommendations/diagnosis? Do you have access to an ask-a-nurse/physician hotline? Many large local hospitals offer this service. You may be able to get a second opinion that way. I don't understand the no orange juice either. Maybe the citrus is irritating, but I would think he would say to eliminate other citrusy/acidic foods.
My son complain of this a few weeks ago. I told him to drink lots of water because sometimes when urine is really strong (dark yellow color) it can be painful to release. I also got him some cranberry juice because that seems to have some healing properties for bladder-related issues. It seemed to do the trick for him.
In regards to collecting a stool sample, our doctor said to place a paper towel over the toilet before your son goes because it makes it easier to collect and won't get diluted by the stool water. I hope you get some answers soon and the problem clears up quickly.Especially in time for his birthday! Poor guy!

Sorry your little guy is not feeling well. Lots of fluids and cranberry juice do help with bladder problems. Have your husband add you to the list of people that are allowed to call the doctor's office and get information. It is a HIPPA form that lists who is permitted to receive information on the patient. The only issue might be if this conflicts with the information in the child custody papers. It is ok to have all three of you on there, but perhaps the mother can also give her approval to have you added to the list at the doctor's office. You husband needs to take a break at work and call the doctor's office for the results of the urinalysis and see if he should be on an antibiotic or what follow up is required.

ETA: Poor kid! Get dad to call his ex NOW! This child could be in extreme pain if it's kidney stones!!! This child's health needs to become a priority right now.

From what I understand, it's very rare that boys get UTIs.
Please don't give him medicine without the peds approval and for goodness sake, could a parent PLEASE call the doctors office and get some actual information, not just "no oj"!!??

If you're not satisfied with the doc's diagnosis (or lack thereof), try contacting the insurance people (or his pediatrician) for a referral to a pediatric urologist. Good luck to the poor little fella.
